What is Support Co-ordination?

Support Co-ordination is a service for NDIS participants that enables them to put their NDIS plan into action and make the most out of their NDIS supports and services. Support Co-ordination can assist people to understand their NDIS plan, implement their support and link or engage with informal, mainstream, community or funded supports.

Through our Support Co-ordination, we’re committed to supporting people to live life to the fullest with individual assistance tailored to your situation and built around your dreams and goals for the future. Our vision for every person we support is to see them achieve a full, meaningful and inclusive life, whatever that may be for each individual.

CALDS Support Co-ordination can:

  • Help you understand the NDIS and your NDIS plan
  • Assist you to access the opportunities your NDIS plan offers
  • Assist you to identify support providers who could be a good fit for you to achieve the goals in your NDIS plan

Our Support Co-ordination service is available right around South-East Queensland and Toowoomba, and is a mobile service that can come to you in your home or in the community.
